Background and/or Objectives: A comprehensive review of current literature was done in order to assess the clinical use, application and effectiveness of PRP and exosomes and their biological properties.

Design: A systematic literature review was conducted utilizing MEDLINE, PubMed, and Cochrane Library. Search terms included “PRP,” and “Exosome therapy".

Setting : Community teaching hospital.

Participants : N/A

Interventions: PRP and exosome therapies across various medical conditions, including osteoarthritis, tendinopathies, spinal disorders, and wound healing.

Main Outcome Measures: Clinical efficacy (pain reduction, functional outcomes, healing rates)
Safety profile (risk of immune response, adverse effects, regulatory concerns)
Patient preference factors (cost, accessibility)

Results: PRP has been extensively studied for treating osteoarthritis, tendinopathies, and intervertebral disc degeneration, showing significant pain reduction and functional improvement. One study found PRP to be more effective than hyaluronic acid for knee OA, and preliminary studies suggest its role in discogenic back pain relief. Exosome therapy, particularly mesenchymal stem cell-derived exosomes, has demonstrated promising preclinical results in cartilage repair and neural regeneration by promoting extracellular matrix restoration and inhibiting apoptosis. PRP-derived exosomes have been shown to enhance chondrocyte proliferation and inhibit apoptosis more effectively than PRP alone. Regarding safety, PRP is generally well tolerated due to its autologous nature, but potential side effects include local inflammation, infection and rare cases of nodule formation. Exosomes, being acellular, have low immunogenicity, making them safer for repeated administration. However, concerns exist regarding standardization, potential immune reactions, and regulatory approval. Patient preference leans toward PRP due to its cost-effectiveness and accessibility, while exosome therapy remains costly and faces regulatory challenges due to low yield, complex isolation process, and quality control issues. Emerging research suggests promising results when combining PRP-derived exosomes with cyclic peptide-modified β-TCP scaffold.

Conclusions: PRP is well-established, cost-effective, and accessible, making it a preferred option for musculoskeletal conditions. Exosome therapy, though promising, requires further standardization and regulatory approval to enhance clinical applicability given.
