Background and/or Objectives: To investigate the epidemiology of injuries in Boys’ high school contact and collision sports including football, soccer, wrestling, ice hockey, and lacrosse.

Design: Descriptive epidemiology study

Setting : National High School Sports-Related Injury Surveillance Study (also known as High School RIO) data from 2013-14 through 2022-2023 academic years.

Participants : High school athletes participating in boys’ contact and collision sports

Interventions: NA

Main Outcome Measures: Characteristics of injuries sustained in boys’ contact and collision sports, including time loss ( < 1 week, 1-3 weeks, >3 weeks), body site injured, diagnosis, and injury severity (measured as a time loss of >3 weeks) were examined using frequencies and percentages. Athlete exposures (AE) were used to calculate injury rates per 1,000 AEs.

Results: Football had the highest rate of injury in competition (13.56 injuries/1000AE) and practice (2.19 injuries/1000AE). Most sports had injuries with a time loss of < 1 week (43.5%-50.2% depending on sport), except for ice hockey with the most time loss being 1-3 weeks (48.5%). The most common injured body part was the head/face (24.1%). The most common diagnosis was ligament sprains (24.3%) followed by concussions (22.0%).

Football had the highest severe rate of injury in competition (1.64 injuries/1000AE) and practice (0.25 injuries/1000AE). Rates of severe injury were higher in competition than practice for all sports. Among severe injuries, the most common injured body part was the knees (26.3%) and the most common diagnosis was fracture (30.3%).

Conclusions: The majority of injured high school boys engaging in contact and collision sports returned to sport within 1 week. The most common injuries were to the head/face and the most common diagnosis was ligament sprains. For injury prevention, physiatrists can consider recommending neuromuscular training during warmups and prophylactic bracing. However, further sports-specific research is needed.
