Background and/or Objectives: Adults with cerebral palsy (CP) experience a lifelong spectrum of comorbidities, including musculoskeletal and neurological symptoms. This study is aimed to identify clinical features and risk factors of degenerative changes in cervical spine in adults with CP.

Design: A longitudinal study.

Setting : A teaching hospital.

Participants : A total of 26 participants were included in this study The study population included 17 (65.4%) males, and 9 (34.6%) females, with a mean age of 49.8 (39-76). The distribution of the current Gross Motor Function Classification System (GMFCS) level was as follows: Level I, 3 (11.5%); Level II, 2 (7.7%); Level III, 3 (11.5%); Level IV, 16 (61.5%); and Level V, 2 (7.7%).

Interventions: None

Main Outcome Measures: Torg-Pavlov ratio (TPR), presence of cervical myelopathy in imaging studies

Results: Thirteen (50%) participants experienced GMFCS change. Twelve out of 26 (46%) participants were identified with cervical myelopathy. TPR was calculated in 16 participants at C3 and C4, 13 participants at C5, 10 participants at C6. Mean value of TPR was as follows; C3, 0.72 (0.50-0.91); C4, 0.78 (0.52-1.19); C5 0.82 (0.59-1.17); C6, 0.74 (0.60-0.90). 17 (65%) participants had severe cervical canal stenosis, at least one level. GMFCS change was negatively correlated with the TPR at C4 (correlation coefficient -0.73, p-value = 0.04).

Conclusions: If the GMFCS is deteriorated in a person with CP who has severe cervical spinal stenosis, cervical myelopathy should be considered.
