Efficacy of PRP Treatment for Glenohumeral Osteoarthritis: A Multicenter Analysis of Patient-Reported Outcomes

Background and/or Objectives: Platelet-rich plasma (PRP) is a promising alternative for glenohumeral osteoarthritis (GH OA), given its effectiveness in treating hip and knee OA. However, existing studies are limited by small samples and short follow-up. This study presents a large-sample, 12-month-long cohort analysis of patient-reported outcomes following PRP for GH OA.

Design: We analyzed diagnostic profile, total delivered platelets, and patient-reported outcomes from a nationwide regenerative medicine database (2019–2025). All changes were assessed using the Wilcoxon signed-rank test. Minimal Clinically Important Difference (MCID) thresholds were calculated using the distribution-based method (0.5 × baseline SD) to assess the clinical significance of score changes.

Setting : Retrospective cohort study

Participants : All cases had a GH OA diagnosis based on clinical and/or radiographic criteria.

Interventions: Single or multiple PRP injections.

Main Outcome Measures: Numeric Pain Rating (NPR), Desired Functionality (DF), Outcome Success Index (OSI), and QuickDASH (QD)

Results: A total of 295 cases (327 sessions) were reported. Most patients had pain >1 year (177, 60%) and had tried prior treatment (184, 62%). Fifty-six sessions included platelet cell data, with an average maximum total platelet count of 6.57 × 10⁹ ± 2.72 × 10⁹. At 12-month follow-up, median change from baseline vs MCID was: NPR -2 (IQR: -4 – 0, p < 0.05) vs -1.15, DF 10 (IQR: -8.3 – 29, p < 0.05) vs 11.8, OSI 0.41 (0 – 0.7, p < 0.05) vs 0.32, and QD -13.7 (IQR: -25 – 2.3, p < 0.05) vs -7.56.

Conclusions: Statistically significant improvements were observed across all patient-reported outcomes at 12-month follow-up. Median changes met or exceeded MCID thresholds for NPR, OSI, and QD, indicating clinically meaningful benefit. Despite PRP formulation variability, treatment with >6 billion delivered platelets was associated with improved pain and function.
