Microfragmented Adipose Tissue and Platelet-Rich Plasma Treatment for Lateral Collateral Ligament Partial Tear: A Case Report

Case Diagnosis: A 28-year old male jiu-jitsu athlete presents with a 3-week history of right knee pain (7/10 pain scale rating) and instability after sustaining an injury from a rolling maneuver who is diagnosed with high-grade partial tear of his lateral collateral ligament (LCL).

Case Description or Program Description: Physical exam revealed fibular head prominence with increased translation, tenderness to palpation (TTP) at distal LCL and moderate varus laxity compared to contralateral side. He underwent diagnostic ultrasound evaluation by a sports medicine physician, which revealed hypoechoic thickening of the right LCL distally with loss of congruency, consistent with high-grade tear. Magnetic Resonance Imaging (MRI) revealed intrinsic edema and attenuation of fibers within the LCL, confirming high-grade partial tear. Microfragmented Adipose Tissue (MFAT) and Platelet-Rich Plasma (PRP) were recommended as treatment options.

Setting: Outpatient Clinic

Assessment/Results: 6 weeks after receiving ultrasound-guided MFAT and PRP treatments to the LCL, he reports improved pain (2/10) and stability. After 12 weeks, minimum varus laxity and no TTP were found, with continued improvement in stability. After 16 weeks, the patient reported rolling, squatting, and performing deadlifts without instability or pain. After 21 weeks, ultrasound revealed increased thickness, echogenicity, and improved integrity with varus stress.

Discussion (relevance): The use of MFAT with PRP for treating a high-grade partial LCL tear shows promise as a nonsurgical approach to ligament injuries. This minimally invasive technique offers rapid recovery and return to activities, bridging the gap between conservative management and surgery. Notably, the patient improved without physical therapy, addressing a common confounding variable in regenerative medicine studies.
Further research is needed to validate these findings in larger populations, compare outcomes with other treatments, establish optimal protocols, and assess re-injury potential long-term.

Conclusions: As regenerative medicine continues to evolve, this case highlights the potential for minimally invasive approaches in the management of ligamentous injuries.
